Students in Ohio are required to receive the meningococcal vaccine (MCV4) for entry into grades 7, and 12. Your child may have received one dose of this vaccine; however, there is a two-dose requirement if the first dose was given before age 16. Students without proof of vaccination will be subject to exclusion from school.
The following vaccines are available at Allen County Public Health or with your PCP:

Meningitis (MCV4)-Required for both 7th and 12th graders. One or two doses, depending on age, to protect against meningitis) -Teens are at increased risk for this bacterial infection. Meningitis can kill a healthy person in less than 48 hours. Survivors can have serious problems that last the rest of their lives. Most colleges require this vaccine for those students who live in dorms.

Pertussis (Whooping Cough) (Tdap) Required for ONLY 7th graders. One dose to protect against Diphtheria, Tetanus, and Pertussis)-This disease is spread by coughing and sneezing. Pertussis has been increasing in recent years; therefore, it is important to be up-to-date with vaccination. Teens can be sick for weeks with pertussis.

Incoming 7th GRADE: In accordance with the Ohio Department of Health all incoming 7th graders are required to have the following two immunizations prior to start of the year: Tdap (Adacel or Boostrix) & Meningococcal (Menactra)
Incoming 12th GRADE: In accordance with the Ohio Department of Health, all incoming 12th graders are required to have the following immunization prior to the start of the school year: Meningococcal (Menactra)
Kindergarten: In accordance with the Ohio Department of Health all incoming kindergarten students are required to have the following immunizations prior to the start of the year: 1. DTap 2. Polio 3. MMR 4. Hep B 5. Varicella (chickenpox)
To assist parents in meeting these requirements an appointment can be made with the Health Department either by phone or on-line.
