Your child's health and well-being are important to us.  If you have a change in address, phone number or a change in your child's medical condition, please let the school know as soon as possible.

Month of October/November/December:

The nursing staff will be screening all 1st , 3rd and 11th graders for vision and hearing per ODH guidelines. If your child does not pass, a referral letter will be sent home.

Dental Outreach Program-Health Partners Coming to Elida in March! 

Health Partners of Western Ohio will be providing dental treatments to Elida students K-5th grade next March. (exam, cleaning, fluoride, and sealants) Parent permission forms will be going home soon and the deadline to return the forms is September 9. More information

Required Immunizations for 2022-23 School Year 

* Please provide a copy of the immunization record to the school after the student has received the shots. This documentation must be received prior to the first day.  As with other school immunizations, students who do not provide documentation of the shots can be excluded. If no documentation is received your student is subject to exclusion from school. 

Incoming 7th GRADE:  In accordance with the Ohio Department of Health all incoming 7th graders are required to have the following two immunizations prior to start of the year:   Tdap (Adacel or Boostrix)  & Meningococcal (Menactra)

Incoming 12th GRADE: In accordance with the Ohio Department of Health, all incoming 12th graders are required to have the following immunization prior to the start of the school year:   Meningococcal (Menactra)

Kindergarten:  In accordance with the Ohio Department of Health all incoming kindergarten students are required to have the following immunizations prior to the start of the year:  1. DTap   2. Polio  3. MMR  4. Hep B  5. Varicella (chickenpox)
